Procedure from Check-in to Consultation / Examination

Check-in at the Revisit Registration Machine

  1. Insert your Patient ID Card into the revisit registration machine located in the center of the lobby.
  2. Your scheduled department for the day will be displayed. If the information is correct, tap “Confirm” at the bottom right of the screen.
  3. Retrieve your receipt and Patient ID Card from the machine.
  4. Place the receipt in the blue file next to the registration machine.

If You Are Using a Loaner Smartphone

  1. Visit the Loaner Smartphone Corner near the escalators on the 1st or 2nd floor of the outpatient building to borrow a device.
  2. Bring the loaner smartphone and proceed to the consultation/examination reception desk.

If You Are Using Your Own Smartphone (My Smartphone)

  1. After completing check-in at the revisit registration machine, proceed directly to the consultation/examination reception desk.

*Using your own smartphone requires prior registration of the dedicated app on your device.

  • If you forget your patient ID card, you will not be able to use the revisit registration machine.
    Please fill out “Hospital Application Form” and place it in the “Revisit Box” at the Central Reception counters [5] and [6] for “Returning Patients”.
    You will be called in the order of submission, so please wait nearby.
    After 11:00 a.m., you do not need to submit the form.
    Please speak directly with the staff at the Central Reception counters [5] and [6].

My Number Card / Eligibility Certificate

  • Use the facial recognition card reader if you have a My Number Card.
  • If not, submit your Eligibility Certificate at your department reception.
  • Publicly funded and welfare certificates must also be submitted at the department reception.

Important:
If you do not present your My Number Card or Eligibility Certificate, you may be charged the full cost of your medical care.
