An official academic transcript will be sent to your university by post in early October for yearlong students and in late April for single semester students. No request form is needed. Please note that HUSA Program will not issue an official transcript before this for system reasons.

Please send the form below to request the Certificate to the Global Initiatives Group.
If you need the certificates to be enclosed in a sealed envelop, please request us to do so.
After receiving the certificates, it is your responsibility to send those documents to the location/ person who requested them.

Please fill out the Request Form for the Academic Transcripts and send the form as an attached file by e-mail with a copy of your ID to the Global Initiatives Group [e-mail address is listed below].

If you request the document to be delivered, please send the shipping fee to our addres by the paymant option as below. The shipping fee will be 1,000yen to 2,500yen, we will inform you after we receive your request.
