Please fill in the application form for return patients and place it in the [Return Patients] (再診) box at the general reception desk Nos. 5 and 6.
After 11:00 am, if you would like to see a doctor who is in the dental department, please speak to our staff at the general reception desk Nos. 5 and 6.

We call you in the order of receiving the application forms for return patients. Please wait around the general reception desk with your Japanese health insurance card, a medical referral letter and your hospital card.

NOTES:
If you do not present your Japanese health insurance card, you have to pay the full amount of medical costs.

If you would like to consult with our doctor during hospitalization in another Japanese medical institution, you should ask them and bring a medical referral letter IN ADVANCE. In this case, your medical cost may change.

Since there are non-consultation dates or the doctor has a previous engagement(s), regardless of whether you have a medical referral letter or not, you may not receive a consultation. Therefore, please confirm beforehand by e-mail if a consultation is possible.
