This notice is for those who take the On Campus Entrance Interview Examination for International Applicants in February.

Applying for a Make-up Examination for the On Campus Entrance Interview Examination for International Applicants in February (Updated at February 4, 2022)

 Of those applicants who have applied for the On Campus Entrance Interview Examination for International Applicants in February, those who fall under any of the following shall be eligible for the make-up examination.

 If you wish to apply, please contact the following contact address and submit the required documents within the application period.

 Please note that applicants who have taken an examination for even one subject on the examination day cannot apply for the make-up examination.

 

Eligible applicants

 (1) Applicants who are infected with COVID-19, and have not yet recovered

 (2) Applicants who are deemed by the health center, etc. to have been in close contact with a person infected with COVID-19, and have been instructed to stay at home

 (3) Applicants who have symptoms of COVID-19 such as a fever of 37.5°C or higher and a cough within the past 14 days prior to the examination day (including the examination day)

 (4) Other applicants permitted by the university to take the make-up examination due to a reason related to COVID-19

 

Required documents

 Applicants who fall under (1) or (3)

  A photocopy of the Examination Card which you printed out via UCARO and a medical certificate from a doctor stating the treatment period, etc.

  *If it is difficult to submit a medical certificate due to infection risk or tightness in the healthcare delivery system, please contact the following address.

 Applicants who fall under (2)

   A photocopy of the Examination Card which you printed out via UCARO with the following information from 1) to 4) handwritten by the applicant in the blank space in the bottom half of the photocopy

    1) The name and the prefecture of the health center that determined that you

                had been in close contact with a person infected with COVID-19

    2) The day when you were informed by the health center that you

                had close contact with a person infected with COVID-19

    3) The period during which you have been instructed to refrain from going out

                unnecessarily as a health observation period

    4) The result of a PCR test you took according to the instruction of the health

                center or the result of a quantitative antigen test administered by the quarantine station

 Applicants who fall under (4)

   A photocopy of the Examination Card which you printed out via UCARO, additional documents that the university will require you to submit, etc.

 

Application period

     From February 22, 2022 at 9:00 to February 25, 2022 at 17:00 (JST)

   * Applications will not be accepted on February 23 due to a national holiday.
