Department of Orthopaedic Surgery

Introduction of department

Specialized medical services

The Department of Orthopaedic Surgery of Hiroshima University Hospital treats disease and trauma to the motor organs. Our department is one of the most qualified orthoapedic programs and fully equipped to treat each and every orthopaedic and rheumatologic condition, injury, or disorder. We also focus on the development of novel therapeutic strategies that are based on biological principles with an aim of improving the patient‘s quality of life, which is greatly impacted by the functionality of the musculoskeletal system.

Target Diseases

The Department of Orthopaedic Surgery has leading specialists that address a broad spectrum of orthopaedic conditions and procedures. They have sub-specialties and provide the most advanced and cutting-edge treatments in their specialized fields;

Knee : Prof. Nobuo Adachi is the chief of the knee surgery group (approx. 10 knee surgeons). Our team of knee surgeons provides a comprehensive range of orthopaedic care, treating patients with acute and chronic sports knee injuries (ligament, cartilage, meniscus injury, etc.) as well as osteoarthritis of the knee.

Oncology: Our group especially focuses on malignant bone tumors and soft tissue tumors.

Hip: All of adult and children’s hip diseases are treated. Our group especially focuses on joint preservation including aggressive conservative treatment and joint-preserving surgery such as osteotomy and arthroscopy except total hip arthroplasty.

Hand (and reconstructive microsurgery): Our team specialize in the following: Anomaly in the hand/ tendinitis/ arthritic change/ nerve compression disorder/ fracture or joint injury affecting the hand, wrist and elbow/ Dupuytren’s contracture/ Hand, wrist and elbow arthroscopy/ soft tissue injury in extremities/ reconstruction following trauma and tumor resection with using microsurgery.

Shoulder: Our group especially focuses on rotator cuff tear, frozen shoulder, recurrent shoulder dislocation, shoulder labrum injury, shoulder arthroplasty, proximal humeral fracture, clavicle fracture, acromio-clavicular joint dislocation.

Foot: Our group especially focuses on lateral ankle ligament, osteochondral lesion, hallux valgus, osteoarthritis, rheumatoid arthritis, flat foot deformity.

Research content

Knee : Our team of knee surgeons has a special interest in knee ligament reconstruction (ACL, PCL, MPFL, etc.), repair of the articular cartilage defects (tissue-engineered cartilage implantation, etc.), and procedures for osteoarthritis of the knee. We have published many clinical and experimental research studies in English medical journals every year.

Oncology: Limb sparing surgery, intraoperative MRI supported surgery, and MRI/US fusion system.

Hip: Development of less invasive technique of hip arthroscopy, Exploration of extraarticular lesion, Mechanism of postoperative dislocation after total hip arthroplasty, Research of motion analysis on hip disorders, Improvement of MR imaging.

Hand: Peripheral nerve regeneration by a new type of conduit/ Development of a new limb perfusion system to preserve the amputated limb and vascularized free tissue transfer/ Ultrasonographic blood flow evaluation for successful free tissue transfer/ Development of a new treating concept using Balloon catheter for trigger finger and hand fracture/ Investigation of pathology of lateral epicondylitis.

Shoulder: Regeneration of rotator cuff tendon, treatment of massive rotator cuff tear, throwing biomechanics
glenoid surface stress change after surgical treatment.

Foot: Articular cartilage repair, and the analysis of kinematics of foot and ankle.

Treatment achievements

Knee : We perform a number of knee surgeries as follows: Ligament reconstruction (ACL, PCL, MCL, PLC), patellar stabilization (proximal and distal realignment), knee replacements (TKA, UKA), tibial and femoral osteotomies, repair of the articular cartilage defects (tissue-engineered cartilage implantation, mosaic plasty, etc.), meniscal repair and resection, synovectomy, etc. In addition, we have treated many professional athletes, returning them to elite function after injury.

Oncology:100-150 operations per year. About 1/3 were for malignant musculoskeletal tumors.

hip: Total hip arthroplasty:90 cases, Rotational acetabular osteotomy:12 cases, Transtrochanteric rotational osteotomy:4 cases, Intertrochanteric varus osteotomy: 5 cases, Arthroscopy: 5 cases, Pelvic fracture 16 cases (in 2017)

Hand: Hand team performed more than 300 operations in 2017. Among them, more than 30 operations were related to microsurgery. Microsurgery (replantation surgery, free tissue transfer) is the greatest features of our hand team, and the success rate after free tissue transfer revealed 100% in 2017.

Shoulder: Rotator cuff repair about 50, frozen shoulder about 20, recurrent shoulder dislocation about 5, fracture around shoulder about 50, shoulder arthroplasty about 10, shoulder labrum injury about 5

Foot: Repair of lateral ankle ligament, low tibial osteotomy, correction of hallux valgus, total ankle arthroplasty, correction of flat foot deformity, arthroscopic surgery for articular cartilage injury
